What we collect

When you contact us or request an appointment, we collect the information you provide, such as your name, email, phone number, scheduling preference, message, and optional uploaded file.

How we use it

We use submitted information to review your request, contact you, coordinate next steps, manage availability, and maintain an internal record of care-team communications.

Optional files

Videos and documents are stored privately and are available only through the authenticated care-team portal. Do not upload government identification, payment-card information, or anything unrelated to your care request.

Important limits

Website forms are not for emergencies, urgent symptoms, or medical advice. A submission does not create a physician-patient relationship or confirm a dialysis session.
